Health insurance for foreigners in the Czech Republic: public or commercial?

Moving to the Czech Republic, studying here or extending your stay? An overview of when you belong to public health insurance, when you need a commercial policy and what to ask before signing.

When you move to the Czech Republic, health insurance is one of the first things to arrange. Without the right document, a visa or residence application often fails, and without insurance even an ordinary doctor visit can become expensive.

The most common question is: do I belong to public health insurance, or do I need a commercial policy? The answer is decided not only by citizenship, but mainly by your residence status, employment and other legal conditions.

Public health insurance: who participates

Participation in the public system can be established by permanent residence, certain employment or special protection regimes. Long-term residence of an adult alone does not by itself guarantee participation in public insurance.

If you belong to the public system, premiums are paid under public health insurance rules and your health insurance company covers your care. Always confirm your status directly with the health insurance company.

Commercial insurance for residence

For some long-term visas and residence permits you need to document travel health insurance. The scope of necessary and urgent care or comprehensive care also depends on where you apply and on the length of your stay.

Verify the required document and its particulars against the current guidelines of the Ministry of the Interior. Requirements may differ by application type and not every policy is acceptable to the authorities.

Children and family

Minors with a long-term residence permit have been included in public health insurance since 2024. A long-term visa alone, however, is not the same as a long-term residence permit.

Confirm the child’s registration and premium payments directly with the health insurance company. For families it makes sense to arrange cover for all members at once so that the policies follow on from each other.

Temporary protection has special rules

Temporary protection establishes special public health insurance rules. First confirm your status and premium obligations with the health insurance company; commercial insurance is not automatically mandatory for every foreigner.

What to ask before signing a policy

  • Scope of care: necessary and urgent, or comprehensive care?
  • Exclusions, waiting periods and any co-payments.
  • Policy conditions for pregnancy, childbirth, children and chronic conditions.
  • Which documents the insurer issues and whether the authorities accept them.
  • Continuity of cover when extending your stay.

How much health insurance for foreigners costs

The price of commercial insurance depends on age, length of stay, scope of care and policy conditions. There is no universal price and automatic acceptance cannot be promised; your specific situation needs to be reviewed first.

Conclusion

This article is a general overview, not a confirmation of entitlement or document acceptability. Verify current requirements and exceptions with the Ministry of the Interior and the relevant health insurance company.
